8/6/2017 · Payment for the postoperative, post-discharge care is split between the surgeon who agrees to the transfer of care and the physician/QHP who agrees to accept the transfer of care. As stated previously, this transfer of care agreement may be in the form of a letter or an annotation in the discharge summary, hospital record, or ASC record.
